Painful Cramping a good or bad sign?

My period was due today no show as yet. Test all came back positive but I've been cramping since Monday and feel really achy down there. This is baby no4 but I don't remember this pain and aching with other pregnancies., I'm expecting af every time I go to toilet I'm not sure wat to think x has any one had constant cramping pains with painful cramp coming and going in waves. I'm really worried it may be eptopic (Dr won't give me a scan till 6weeks min) has anyone experienced anything like this and what was your out come x thanks in advance xx
