Question

IUI- 8 MATURE, RE WANTS TO TRIGGER?!

Hi all,

I'm 5 days into stimming (tonight will be day 6) for my first IUI, with follistim and ganirelix. I started at 150 for 3 days and was lowered to 75 after responding so quickly. My RE was shocked that I responded so quickly and produced so many growing follicles- today I had 4 on my left ovary that were 17mm, and 4 on my right ovary ranging from 16-18mm. This terrifies me but he wants to go ahead and trigger tomorrow!! His partner agreed that it wouldn't be too risky.

I'm SO worried that this is too many- has anyone ever triggered with so many follies for IUI and not ended up with multiples???

Oh yeah, I'm 28 and have PCOS, in case that matters

In order for all these follicles to be mature they not only have to be of the right size but also must have the appropriate estrogen level, an E2 of 200 or more per mature follicle. Do you know your latest E2 number? It could be that according to the bloodwork there are fewer follicles mature than you think.

Following... I am on my last day of Clomid and I go in this Friday to see how my ovaries are maturing and if I will have to take the Bravelle (FSH) shot or go straight to the Ovidrel (Trigger) shot. But they also want me to take Prometrium (Progesterone) vaginally and I am not sure when they want me to do that. This is the first month of doing timed intercourse so it is all new to me. Mozart- You're right, and I definitely am keeping track of my estrogen. Maybe that will be the deciding factor. It was 20 at baseline (so low...) and then 386 after 3 days of stims (with 8 follies between 11-14), so if it's anything higher than 1000-1200 with today's 8 large follicles, I will probably be looking at at least 5-6 mature. I'm waiting for the call to see what today's estrogen was!!

Muffet- Thank you and good luck to you too! They didn't monitor you more closely with clomid? I've never done climid, but I would have thought maybe they'd do more than one ultrasound. It's crazy how this is all such a guessing game!!

I am not an expert with PCOS but I have read that egg quality is often lower when dealing with this condition. This could very well be the reason why your dr is not concerned about several mature follicles. Good luck with everything. Fx!

Mozart- I've read that too :(- I actually got pregnant naturally on my first time ovulating since trying (3rd month "trying" but first time actually ovulating) and had a miscarriage due to monosomy x. So, although most monosomy x's are apparently caused by bad sperm, some are bad eggs and my RE thinks that was the case with me. Oh well, we shall see. Thanks!

So my estrogen was 1120...eek! Probably 5 mature. Eek!!! So torn...

Hi KayCH28, this is my progress so far:
After a very long waiting process I finally had follicles that are maturing, I have 3 on my left side that are ranging in sizes 20.2, 19.1 & 15.3 and on my right side I have 2 ranging in sizes of 20.1 & 17.8. my estrogen came back as 952 but my LH is at 9.67. We did the Ovidrel (Trigger) shot last night after we DTD, even though he was not feeling up to it. I go back to the doctors for the pregnancy test on July 27th, I am hoping this will be our month for the BFP, I have been dreaming of for so long now. Please send lots of baby dust and a sticky bean my way.

That's great! With that E2 level you're probably looking at 4 mature follicles. I feel like that's the perfect number for IUI and I wish you lots and lots of luck! I triggered last Friday, but we didn't do IUI- just intercourse but I purposely didn't time it great since I was so scared of multiples with 8 mature follicles. I ended up getting moderate OHSS, which is no fun. :(

KayCh28, yeah we are not doing the IUI either just timed intercourse. I have tried to find out if 952 was a good estrogen level but nothing is really saying much. I have been trying to see when I should actually drop the egg since I did the shot last night and before the shot we DTD. My main reason for wanting to know is because I feel absolutely no different at all. Please keep me posted on what happens with you. Good luck and sending baby dust your way...

That's actually a great estrogen level for the amount of follicles you have. Most mature follicles produce between 200-250 estrogen (according to my RE- and google seems to agree, though some sites give a broader range). I would definitely DTD again today if I were you, just to be safe. If you triggered last night, you will probably ovulate tomorrow morning or so. Apparently most people ovulate about 36 hours after the trigger, but anywhere from 24-48 can be normal. I usually feel ovulation, but this time I released 8-10 follicles and did not feel a THING! Crazy! So you might not feel anything! :)
