Question

BLOOD RESULTS - CAN ANYONE SHED SOME LIGHT

Hey guys. We have been TTC for 2.5 years now and I am so frustrated and sad. These are my results:

Day 3 Oestradiol 95
follicular 45-600, mid cyc 300-1800 luteal 160-
780 U/L

LH 5

FSH 11

Day 21

Oestradiol 188

LH 5

FSH 6

Can anyone tell me please if these results are ok. Do I need to take clomid or something similar. I am 44 now so feel like Im running out of time.

Your FSH is on the high side, but for your age, it's in the normal range. Other than that, your numbers look fine to me (not a doctor).

My doctor doesn't recommend clomid for women of advanced maternal age as it can thin the lining, which is generally already on the thin side as we age. And if you ovulate on your own, it's probably not going to do much for you anyway, unless you add in a trigger shot.

At 44, my doctor said only 1 egg in 10 is genetically sound. So while your chances each month are low, there are good eggs still out there. It's just a matter of finding one. IUI or IVF will up your chances slightly - to about 1-5% for IUI and a teensy bit higher for IVF, maybe 5-10%. IVF with PGD testing might be a better bet, as you can wait until you get a good embryo - but with such a high FSH, you will likely need high doses of stims. You didn't mention your AFC or AMH. IVF with donor eggs will up your odds considerably to nearly 50-60%. Good luck!

Thank you so much for answering my question. I have a big list of results here and cant find AFC or AMH on it but thats not to say i didn't miss it. Can I just ask you what is a trigger shot. Im so useless when it comes to this stuff.

A "trigger shot" is an injection of HCG (the pregnancy hormone) used to make you ovulate. Different reasons for doing one: is if you do "stims" (medications to increase the number of follicles you mature), a trigger shot will cause you to release all of the mature eggs and not just the one from the lead follicle; to better pinpoint timing for procedures like IVF and IUI; to cause ovluation in women who don't ovulate.

AFC stands for antral follicle count. AFC is done by ultrasound on day 3 of your cycle to look at your ovaries and how many potential follicles you have at the starting gate. This gives you an indication of your ovarian reserve and combined with your FSH levels can tell your doctors how well you will respond to stims.

AMH stands for Anti-Mullerian Hormone and is a blood test that also will indicate ovarian reserve. The higher the number, the better. At 44, I would expect your number to be less than 1.

Thanks so much. You are very knowledgeable. Even after 2.5 years, I am still in the dark about a lot of this stuff. I am very interested in the trigger shot. I never heard of it before. Is it just one shot, how do you take it or when do you take it.

It's just one shot. There are a couple of different brands and strengths. For TI (timed intercourse) and IUI, generally they use Ovidrel, which is 250mcg (approximately 6500IU) and is generally done sub-cutaneously ("sub Q" little needle in the belly) -- although it can be done intramuscularly ("IM" big needle in the butt) as well. For IVF, they generally use novarel or no-name HCG in either 5000IU or 10000IU strength, IM. You would ovulate approximately 36 hours after the shot.

wow. Thanks so much. I was speaking to a friend last night about the whole thing and the info you were giving me and she said she can get her hands on both clomid and HCG so I am tempted to self administer. I did a little research last night and I came across a couple of ladies who did self prescribe both and got pregnant. Im so desperate at this stage as we want a baby so badly and my time is short. Thanks again you have been a real gem. Are you TTC yourself.

I can't advise you to self administer. You have no idea how you'll respond on clomid. I had 4 mature follicles on clomid, so potentially quadruplets. Of course, as I said, clomid can cause lining issues - it made mine be only 4.8, which is way too thin.

You really need to have medical supervision. Clomid and trigger shots are super cheap.

Yes, I'm TTC. I got pregnant in May, but the baby passed away in mid-July, so we're trying again but the odds are so very slim.

oh Im so sorry to hear that. Have you been trying long. How old are you. Are you on any meds. I know its not a good idea to self medicate so Im not sure where to go with it. I have taken clomid before just for 2 months but nothing came of it. I wasnt monitored so I don't know about follicles but I didn't bet pregnant so I presume it didn't do much. I don't know where youre from but here, its not so easy to get prescribed fertility drugs, they make you go through loops and costs a fortune to see specialists before they even prescribe anything for you. I'm 45. We've been trying for almost a year and a half for baby #2 (baby #1 took us 12 years). We're running out of time and options, which breaks my heart. Fertility doctors/meds/procedures cost a fortune here, too (I'm in the US). My last pregnancy was the result of IVF - which cost us about $34,000. My husband isn't willing to do that again, so we're just trying some unmedicated IUIs and then giving up.

Oh no im so sorry and wow that's expensive im in Ireland and some might say its a little backward here. . When I got my bloods done my gp gave me a referral letter for specialist but we didn't go yet as its quite expensive. .. im praying for u and hope u get ur blessing. .
