Negative hpt but positive ultrasound at 4 weeks . Any input please , thank you

I had a negative hpt the day before missed period and the day after I had a doctors appointment he scannned me and said I look 4 weeks pregnant and that the lining is thick and there was an empty egg thing preparing for pregnancy, I just want to know what people’s thought are on this as It would be wonderful news if I was. Its the hpt that makes me think otherwise I have had 5 miscarriages in the last 2 years and I’ve been trying for 4 years, I’m currently on clomid 50mg days 5-11 I’m currently 3 days late now and no sign of af.
