Question

Concerned about the next step

My husband and I have been TTC for 15 months now. I have had blood work done and seem to be fine. My progesterone at 7 DPO was 22.2 (we have been using clomid for the past 2 month). My husband had a seman analysis 3 months ago and he had lower molitly (27%) but a count of 2.76 million with a volume of 4. I know that is a good count, but I'm worried the motility is what's keeping us from getting a BFP each month. Has anyone had any success with lower motility? Is IUI Our best option at this point?

Have you tried supplements to increase sperm quality? IUI can help when male factors are involved but often has to be done for several cycles prior to success. I don't know how old you are but if you are not pressed for time because of your age I would not necessarily jump straight to IVF. What is your dr's recommendation at this point?

I'm 26 and he is 25. The doctor want us to continue clomid for 2 more months, then he wants me to do an HSG (dye test). I'm just getting frustrated and would like to know my realistic options. The constant worry is depressing.

The good news is that you are not pressed for time because of your age. I would do the HSG test as soon as possible. If your problems conceiving stem from blocked tubes, why waste time and money trying with clomid, IUI, etc. It is often difficult to say why it takes some couples longer to conceive. It is frustrating, I fully agree with you. I would do the HSG test for sure and start supplements to improve sperm. Do some research on supplements online, there are quite a few options; then once the HSG test was done try a few rounds of IUI. I hope something will work out for you soon!

I would get your husband something like 'Daily Wellness Company, Fertility Blend for Men' from iherb.com to try and increase sperm health and count. Even with IUI boosting his count would help. You could take the female version, both have folate in them (folic acid). Cut out coffee, alcohol and eat more green leafy vegetables too. Do the HSG, it doesn't hurt and it is good to know you are all good. No harm in trying IUI but the cost :)

My doctor doesn't worry so much about volume but more on count and mainly motility. My husband and I deal with male factor infertility and I've had 3 iui's w/o success. I agree that you should do the HSG before moving on with iui. If tubes are blocked the iui will be a waste anyhow. If you are cleared, then iui may be the best bet for you. At the clinic I go to for iui, the doctor wants to see at least 50% motility. My husband was actually prescribled Clomid to help with his and we've seen improvement. Work on getting his counts up and making sure you're okay before moving forward with iui or IVF. Age is on your side! Best of luck to you and prayers for your BFP soon!

Was the count 276 million or 2.76 million? If 276 million that high count might make up for the low motility since you will still have tons of motile ones. You can probably work on improving the motility and may not need IUI. I agree with others to get the HSG just to make sure everything is clear before you spend a lot of money on treatments. You would definitely be IUI candidates. If 2.76 million that count is pretty low, so I'm assuming that's not what it is. best of luck!

Thank you everyone for the responses. His count was 276 million, not 2.76 million. Sorry for the typo. I'm going to ask my dr if I can do the HSG next month, I'm a teacher and will be on Christmas break. I've heard that the high count could make up for the motility and it could just take longer. I'm getting very discouraged and thinking that maybe I should just take the hint. Maybe we just aren't meant to have children. That thought is so emotionally overwhelming for me because I have always wanted to me a mom and my husband wants to be a dad more than anything. I'm just going to try and keep faith for a while longer and hope for the best.

I really believe you will conceive and encourage you to be hopeful. We suffered from unexplained infertility for 13 years. We finally became successful with ZIFT and after that I conceived naturally at ages 39, 42, and 45....So you never know what God has in mind for your life. Since you are young you have a lot of opportunities coming up for success, just be patient with it. You should not be worried at this point. Best wishes.
