Question

my wife's hcg level is <1.20 mlU/ml. Is she pregnant?

technically speaking, no. But if the level keeps going up, then yes. Women can have very small amounts of hcg in their system at any point in the month even when not pregnant, no they expect a level to be more than 5-25 to be considered pregnant.

but she has missed her periods this month, and this test was done after that?

at 21 they still doubt it ask for a re-test... depending on her age as well there are other factors that can distribute to numbers elevating between 1-5 - without knowing her age I don't want to sound like I am offending - but menopause? Also they normally won't rise too high over 25 until your first missed period. Mine were 21 3 days before my missed period then 190 the day after I missed my period - hope this helps some what and good luck for your journey :-)
