Question

Is it possible that conception happened 5 days after O pain?

I had O pains on cd 15 & 16, but it appears that I didn't conceive until cd 21 based upon my hcg numbers and the fact that there was no sac yet in my ultrasound.

is it possible to have signs of O, but to not actually O until a few days after all the signs (O pain, ewcm, high sex drive)?

yes it is possible, you can start seeing signs of ovulation about 5 days before you ovulate.

The easiest way to tell that you're about to ovulate (for me) is to track my cervical mucus.
The days before you ovulate your mucus will be increased in amount and thick, white/ off white and creamy.
The day of ovulation and possibly the day after your cervical mucus will turn a clear color and it will become really stretchy.

Yes, it's possible. Ovulation pains can appear before, during or after ovulation. I think testing or temping would be the only way of determining which one is the case for you, although from what you're writing here, it seems your pains arrive before OV itself. :)
